The Price of Perfection: Inside the High-Stakes World of Penile Enhancement

Editor’s Note: This review was originally published during SXSW 2026. World of Wonder releases "Manhood" in select theaters starting Wednesday, October 28.

In a modern era defined by the democratization of aesthetic surgery—where Botox and dermal fillers have become as routine as a morning coffee—a new, more intimate frontier of body modification has emerged from the shadows. What if you could increase your penile girth with nothing more than a series of needles and a bit of hyaluronic acid? This is the central premise of Manhood, the latest documentary from filmmaker Daniel Lombroso. By turning his lens toward the burgeoning industry of penile augmentation, Lombroso exposes a world where insecurities are monetized, and the line between medical intervention and body dysmorphia becomes dangerously thin.

The Business of Masculinity

At the heart of the film is Bill Moore, a Dallas-based entrepreneur and self-described "bio-hacking" mogul. Moore, a man who wears his own history of cosmetic procedures like a badge of honor, has patented a minimally invasive technique he calls PhaloFILL. To hear Moore tell it, he is providing a public service: helping men "feel whole again" by addressing a deep-seated, universal anxiety.

"Every man wants a bigger penis," Moore asserts during the film, a claim that serves as the foundation for his lucrative business model. The procedure, which utilizes the same hyaluronic acid commonly found in facial fillers, promises increased girth with minimal downtime. However, as Lombroso’s film meticulously documents, this "lunch-hour" procedure is part of a complex psychological ecosystem where the desire for physical transformation often masks deeper, unresolved emotional voids.

A Chronology of the Quest for "More"

The history of penile enhancement is as old as civilization itself, though it has historically been marked by catastrophe. From ancient, brutal mechanical stretching devices to the ill-advised, often dangerous methods of the past, the quest to alter the male anatomy has long been a pursuit filled with failure.

In Manhood, we see this lineage continue into the present day. The film follows Ruben, a father of five from Arizona, whose story serves as a cautionary tale of the "enhancement trap." Despite his wife’s stated satisfaction with his natural anatomy, Ruben has funneled tens of thousands of dollars into repeated procedures. He is a repeat client of Moore’s, chasing a dream of physical perfection that is constantly moving. The film clarifies a vital medical reality: no doctor has yet successfully, safely, and consistently mastered the art of penile lengthening. While Moore’s fillers can certainly add girth, they do not add length, a distinction that many of his clients—driven by intense dysmorphia—struggle to accept.

The narrative shifts from the clinical to the catastrophic when the film introduces the work of Dr. Loria, a New York City-based practitioner. Unlike the sleek, marketing-forward presentation of Moore’s practice, Loria’s results are depicted with unflinching, stomach-turning detail. The film showcases patients left with "nodules" and deformities—the consequence of improper injection techniques that leave the anatomy mangled.

Supporting Data: The Anatomy of Insecurity

The documentary excels in its sociological analysis of why these men are willing to risk their health, finances, and sexual function. Penis size remains one of the most significant, yet rarely discussed, psychic hang-ups for men across both heterosexual and homosexual demographics.

Within the LGBTQ+ community, where the male form is the primary object of both desire and comparison, the pressure to conform to an idealized, hyper-masculine standard is particularly acute. Lombroso interviews David Smith, a former OnlyFans creator turned nursing student, who illustrates the intersection of digital validation and physical insecurity. For David, his career path required a certain "performance" of masculinity. When his own attempts to fix his insecurities—led by the dubious care of Dr. Loria—went horribly wrong, he was left with what Moore describes as "the penis of an alien."

The film does not shy away from the ethically murky waters of the patient-doctor relationship. When Moore takes David into his own home—and his own bed—as part of a polyamorous arrangement while helping him navigate reconstructive surgery, the audience is left to grapple with the blurred boundaries of care, desire, and exploitation. It is a moment of profound vulnerability that highlights the desperation of the patients seeking these treatments.

Official Responses and Ethical Dilemmas

The film’s power lies in its non-judgmental, observational approach. Lombroso, known for his previous work documenting the rise of the alt-right in White Noise, applies the same rigorous, fly-on-the-wall journalistic technique to this intimate, high-stakes environment.

When asked about the ethics of his practice, Moore remains unmoved by the traditional standards of medicine. He is a merchant of confidence, or at least the appearance of it. During one of the film’s most poignant scenes, he tells David, "I can fill your penis with filler, but I cannot fill the hole in your heart." This admission serves as the film’s thesis: the medical intervention is not a cure for the patient’s internal struggles.

The documentary highlights the lack of regulation in the field of elective, "minimally invasive" penile procedures. Because many of these treatments are classified as cosmetic rather than strictly medical, they operate in a gray area where malpractice insurance is often ignored, and accountability is difficult to enforce. Dr. Loria, appearing on camera, remains unapologetic, framing his work as a service to men who would otherwise be left without options, regardless of the physical outcome.

The Broader Implications: What Does it Mean to be a Man?

Manhood ultimately forces the viewer to confront the changing landscape of masculinity. If we are now in an age where the body is viewed as a "project" to be constantly optimized, where do we draw the line?

The film suggests that the "dick-size complex" is not merely a personal insecurity, but a societal construct fueled by the commodification of the body. When a man feels he must be "fixed" to be worthy of love or sexual success, he becomes a prime candidate for the promises of bio-hackers. The film’s success, however, is that it treats these men with genuine respect. When David finally sees the results of his reconstruction, his tears are not for the aesthetic change alone, but for the relief of having his body "fixed" after a traumatic experience.

Lombroso’s film is a masterclass in tone. It is funny—in the way that human absurdity often is—but it is also deeply sad. It captures the desperation of men who are searching for an external solution to an internal crisis. By the time the credits roll, the audience is left with a challenging question: Is the "manhood" these men are seeking found in the results of a syringe, or in the painful, messy process of accepting oneself?

Manhood is not just a documentary about anatomy; it is a profound exploration of the modern male psyche. It is a necessary, if uncomfortable, look at a trend that is unlikely to slow down as long as the market for insecurity remains as lucrative as it is today.
